Following is the form to nominate your favorite artist for an award.

Please provide the information and suggest an award category from the 7 Artists For A Better World Int “CREATING A BETTER WORLD” Award Categories.

Please note that the AFABW Int Award Selection Committee may deem an artist nominee’s work to be better represented by a different category than the original suggestion.

In the “Artists Information” section below, describe the nominated artist’s activities and accomplishments that relate to the suggested “Creating A Better World” award category. Also include the following:

• How long the artist has been actively helping to create a better world.
• Internet links to supporting documentation of the artist’s accomplishments towards creating a better world.
